Jaina Vallarta, MSN, APRN BC, CCRN, is a board certified Acute Care Nurse Practitioner with over 20 years of clinical experience across diverse healthcare settings, including cardiology, internal medicine, and hospital medicine. She comes to us from Wayne, NJ, a suburb located 20 minutes west of New York City. A graduate of Columbia University’s School of Nursing, she began her advanced practice career at New York Presbyterian Hospital, where she specialized in heart surgery and ventricular assist device management. Since then, Jaina has held leadership roles, including Lead APN for cardiology at St. Joseph’s University Medical Center in Paterson, NJ, where she oversaw advanced cardiac services in heart failure, electrophysiology, and interventional cardiology.
Thirath Chau, MD, MPH is a board certified family physician, healthcare executive, and consultant focused on advanced primary care, startup growth, and value based care. He brings more than a decade of leadership designing and scaling innovative care models that improve outcomes for people with complex chronic conditions.
He has served as chief medical officer at Hopscotch Primary Care and Lyn Health, where he built and expanded advanced primary and virtual care programs for rural and commercially insured populations. Earlier, as regional and senior medical director at Oak Street Health, he led multi site teams across the North Chicagoland region and beyond, reducing hospital admissions, improving quality metrics, strengthening patient and clinician satisfaction, and lowering total cost of care.
