Counties above that line see more prior authorizations and more denials worth appealing; counties below it lean on Medigap rights, which in Nebraska follow the federal rule at 65 (one guaranteed six-month window, no birthday rule, no year-round switching) and, since January 1, 2025 under LB 852, add a guaranteed under-65 disability right to at least Plan A at up to 150% of the age-65 rate (ESRD excluded).
