How Medicare Pays for Your Advocate in Maine

Original Medicare

Since January 2024, Medicare covers professional healthcare navigation (Principal Illness Navigation and Community Health Integration). Part B pays about 80%; a Medigap plan typically covers the rest.

Medicare Advantage

About 58.6% of Maine's Medicare beneficiaries in 2025 are enrolled in a Medicare Advantage plan rather than Original Medicare, more than half of everyone on Medicare in the state. Acceptance varies plan by plan. Tell us which plan you have and we confirm what it covers before any work begins and tell you exactly what, if anything, you will owe.

Not sure what you have?

That is a normal place to start. Read your red-white-and-blue card to us over the phone and we will explain your coverage in plain English, whether or not you sign up.

Maine is different

Four Maine Rules and Programs Worth Knowing

Medicare is federal, but the rules and assistance programs around it change at the state line. These four are specific to Maine, and your advocate works with them every week.

Medigap: community rating and a right to switch

Maine requires community rating for Medigap: premiums cannot vary by age, gender, health status, or claims. If you have kept coverage that supplements Medicare with no gap of more than 90 days since your Medigap open enrollment, you can switch at any time of year to a plan with the same or fewer benefits, from the same or a different company, with no health questions. On top of that, every insurer must accept any applicant for Plan A during a one-month guaranteed issue period it chooses each year. People under 65 on Medicare because of a disability must be offered an insurer's standardized plans during their open enrollment, at the same community rate. And Maine extends the Medicare Advantage trial period: if you join an Advantage plan during your six-month Medigap open enrollment and return to Original Medicare within 36 months, you can still buy any Medigap plan within 90 days after the Advantage coverage ends. Medigap in Maine is regulated by the Maine Bureau of Insurance, at 1-800-300-5000.

Maine SHIP and five Area Agencies on Aging

Maine's State Health Insurance Assistance Program, Maine SHIP, is sponsored by the Office of Aging and Disability Services, the five Area Agencies on Aging, and Legal Services for Maine Elders, at 1-877-353-3771. Its counselors are free and confidential and do not sell insurance. Great for one-off enrollment questions; we point our own members there during Open Enrollment. Maine's five Area Agencies on Aging, Aroostook Area Agency on Aging, Eastern Agency on Aging, Spectrum Generations, SeniorsPlus, and Southern Maine Agency on Aging, are also its Aging and Disability Resource Centers, the front door for meals, rides, caregiver support, and in-home help; the same number, 1-877-353-3771, connects you to the one for your county. The national Eldercare Locator is at 1-800-677-1116. For nursing home, assisted living, and home care concerns, the Maine Long-Term Care Ombudsman Program is at 1-800-499-0229.

Medicare Savings Programs: no asset test, higher limits

Maine law bars an asset test for its Medicare Savings Programs: only your income counts, not your savings, your car, or your home. And the income limits are well above the federal minimums: since July 1, 2024, QMB covers income up to 185% of the federal poverty level, and QI covers income above that up to 250%. QMB pays the Part A and Part B premiums plus Medicare deductibles and coinsurance, and providers are barred by law from billing QMB members for Medicare cost-sharing; QI pays the Part B premium. Qualifying also brings Part D Extra Help with drug costs. The Maine Department of Health and Human Services' Office for Family Independence decides eligibility; apply online through My Maine Connection or call 1-855-797-4357. People who assume their savings or a middle-class income rule them out often qualify, and screening for every one of these programs is what an advocate does first.

Home care: MaineCare Section 19, and no PACE

Maine's Medicaid program is MaineCare, expanded after a 2017 voter referendum. MaineCare's Home and Community Benefits for the Elderly and Adults with Disabilities (Section 19) brings in-home care to adults 18 and older on MaineCare who need a nursing-facility level of care, confirmed by Maine's Assessing Services Agency at 1-833-525-5784; when funding runs short, applicants wait on a first-come, first-served list. You can hire and direct your own attendant, and adult children and other relatives can be paid. A spouse can be paid only through a personal care agency and only for extraordinary personal care, not household tasks, and a guardian, conservator, or the person's representative cannot be paid as a self-directed attendant. Maine also funds Home Based Supports and Services for older and disabled adults, as money allows. Maine has no PACE program. For dual-eligible families we handle the Medicare side while you run the caregiving.

Maine Questions, Answered

Open icon

Yes. Since January 2024, Medicare pays for professional healthcare navigation through Principal Illness Navigation and Community Health Integration services. Medicare Part B covers about 80% of the cost, and a supplemental (Medigap) plan typically covers the rest. We check your coverage and tell you exactly what, if anything, you will owe before your first session.

Open icon

Yes, all 16 counties. The service is fully virtual, delivered by phone, video, and secure messaging, so it works the same in Portland, Lewiston, Bangor, and Augusta as it does on the Midcoast, Downeast, in the western mountains, and up in Aroostook County. Where the nearest specialist is hours away, a virtual advocate you can reach without the trip is often the practical option, not the fallback.

Advocates work virtually and are matched to you, not to a zip code. What matters is that your advocate is the same person every time (One Advocate, For Life), knows Maine's rules like free Maine SHIP counseling, community-rated Medigap and the right to switch to a plan with the same or fewer benefits, the Medicare Savings Programs with no asset test, and MaineCare home care, and gives you their direct phone, email, and text, so you never start over with a stranger in a call queue.

Open icon

Often, yes, and at any time of year. Maine law lets you move to a Medigap plan with the same or fewer benefits, from the same or a different insurance company, with no medical underwriting, as long as you have not gone more than 90 days without coverage that supplements Medicare since your Medigap open enrollment. Separately, every insurer must accept any applicant for Plan A during a one-month guaranteed issue period it picks each year. Maine also requires community rating, so Medigap premiums cannot vary by age or health. Questions and complaints go to the Maine Bureau of Insurance at 1-800-300-5000.

Open icon

Yes, if you are on Medicare because of a disability. Maine law requires every Medigap insurer to offer its standardized plans to people on Medicare through a disability, regardless of age, during their Medigap open enrollment, and to offer Plan A during its annual guaranteed issue month. Because Maine requires community rating, your premium is not set by your age. After that, the same right to switch to a plan with equal or fewer benefits applies to you too. A free Maine SHIP counselor at 1-877-353-3771 can walk you through the options, including Medicare Advantage.

Open icon

Yes, and Maine's rules are specific. MaineCare's Home and Community Benefits for the Elderly and Adults with Disabilities (Section 19) serves adults 18 and older who are on MaineCare and need a nursing-facility level of care, confirmed by an assessment from Maine's Assessing Services Agency at 1-833-525-5784. When funding runs short there is a first-come, first-served waiting list. You can hire and direct your own attendant, and adult children and other relatives can be paid. A spouse can be paid only through a personal care agency and only for extraordinary personal care, not household tasks, and a guardian or the person's representative cannot be paid as a self-directed attendant. Maine also funds Home Based Supports and Services as money allows. Maine has no PACE program. To start, call your Area Agency on Aging at 1-877-353-3771. Our advocacy is billed through Medicare, so we focus on the Medicare side: bills, appeals, appointments, coverage questions. For dual-eligible families that split works well: you manage the caregiving, and your parent's advocate handles the Medicare paperwork.

Maine SHIP, the State Health Insurance Assistance Program, reachable at 1-877-353-3771, offers free, confidential Medicare counseling through Maine's five Area Agencies on Aging and Legal Services for Maine Elders, and it is excellent for one-off questions like comparing Part D or Medigap plans during Open Enrollment. Understood Care is different: a dedicated advocate who does the work itself, makes the calls, files the appeals, untangles the bills, and stays with you month after month, covered by Medicare. The two are complementary, and we often point our own members to SHIP counselors for enrollment-season plan comparisons.

In Maine we are in-network for Original Medicare (traditional fee-for-service Medicare); UnitedHealthcare Medicare Advantage plans (~23% of Maine MA members); Anthem Blue Cross and Blue Shield (Elevance) Medicare Advantage plans (~5% of Maine MA members).
