Medicare is federal, but the rules and assistance programs around it change at the state line. These four are specific to Iowa, and your advocate works with them every week.
Your six-month Medigap Open Enrollment Period begins the first month you are both 65 or older and enrolled in Part B, and it does not repeat. Iowa has no birthday rule and no year-round switching, so miss that window and insurers can medically underwrite you. And unlike a number of other states, Iowa does not require insurers to sell a Medicare Supplement policy to beneficiaries under 65 who have Medicare through disability or ESRD. A few carriers choose to offer under-65 plans, usually at higher premiums, but most do not, so the timing of that one window at 65 is the whole game, which is exactly where an advocate earns their keep.
Iowa's State Health Insurance Assistance Program is branded SHIIP (the Senior Health Insurance Information Program), and it is run by the Iowa Insurance Division with trained, commission-free counselors and volunteers statewide. Statewide helpline: 1-800-351-4664. Counselors take no commissions and sell nothing. Great for one-off enrollment questions; we point our own members there during Open Enrollment.
Iowa does not run its own state prescription-assistance program, so the drug-cost help here is federal: many Iowans qualify for Extra Help (the Part D Low-Income Subsidy) but never enroll. On top of that, Iowa Medicaid runs the Medicare Savings Programs as QMB, SLMB, and QI to help cover Part B premiums and cost-sharing. Apply for those through Iowa Department of Health and Human Services (Iowa HHS), online through the Iowa HHS benefits portal or a local office, or by calling 1-800-972-2017. Screening for every one of these is standard advocate work, and most eligible seniors never claim them.
Iowa's Medicaid alternatives to a nursing facility run through the HCBS Elderly Waiver, which helps Iowans 65 and older who meet a nursing-facility level of care stay home with personal care, homemaking, and other support, administered by Iowa Department of Health and Human Services (Iowa HHS) within Iowa Medicaid (IA Health Link). Its self-direction routes, the Consumer Choices Option and Consumer Directed Attendant Care (CDAC), can pay a qualified family caregiver (an adult child or other relative, though not a spouse or legal guardian). PACE (Program of All-Inclusive Care for the Elderly) bundles all care for adults 55 and older who need a nursing-facility level of care, with Iowa centers run by UnityPoint Health around Cedar Rapids, the Quad Cities, Sioux City, and Waterloo. These are Medicaid programs; for dual-eligible families we handle the Medicare side while you run the caregiving.
