Medicare is federal, but the rules and assistance programs around it change at the state line. These four are specific to Arizona, and your advocate works with them every week.
Arizona has no Medigap birthday rule and no annual guaranteed-issue window, so the federal rules govern: your one guaranteed six-month Medigap Open Enrollment Period begins when you are 65 and enrolled in Part B, and once it closes an insurer can medically underwrite you, raising your rate or declining you, outside a few narrow federal guaranteed-issue situations. Arizona also does not require insurers to offer Medigap to beneficiaries under 65 who qualify through disability, and no insurer currently sells under-65 Medigap in the state; those beneficiaries get a fresh guaranteed-issue window at 65, and Arizona law prohibits charging them a higher premium for having qualified by disability. Because there is no yearly reset here, getting the timing right the first time is exactly where an advocate earns their keep.
Arizona's State Health Insurance Assistance Program is called simply SHIP, run by the Arizona Department of Economic Security (Division of Aging and Adult Services) and delivered by trained, commission-free volunteer counselors statewide. Statewide helpline: 1-800-432-4040. Counselors take no commissions and sell nothing. Great for one-off enrollment questions; we point our own members there during Open Enrollment.
Arizona does not run a state prescription-assistance program, so the drug-cost help here is federal: many Arizonans qualify for Extra Help (the Part D Low-Income Subsidy) but never enroll, and all three Medicare Savings Programs automatically grant it. AHCCCS (Arizona's Medicaid) runs those Medicare Savings Programs as QMB, SLMB, and QI to help cover Part B premiums and cost-sharing, and Arizona applies no asset or resource test to them, so eligibility looks only at income, not savings. Apply through Health-e-Arizona Plus (healthearizonaplus.gov), or by calling AHCCCS at 1-855-432-7587. Screening for every one of these is standard advocate work, and most eligible seniors never claim them.
Arizona delivers Medicaid long-term care through one program, the Arizona Long Term Care System (ALTCS), a managed model run by AHCCCS rather than the PACE programs some other states use. For members who qualify, ALTCS covers in-home care so a person can stay home instead of entering a facility, and its Self-Directed Attendant Care option lets the member hire and direct a family caregiver, an adult child, sibling, or other relative, for attendant and personal care. Unlike California's IHSS, Arizona generally does not pay a spouse, and the caregiver works through a contracted agency after completing training. These are AHCCCS programs; for dual-eligible families we handle the Medicare side while you run the caregiving.
