How Medicare Pays for Your Advocate in Louisiana

Since January 2024, Medicare covers professional healthcare navigation (Principal Illness Navigation and Community Health Integration). Part B pays about 80%; a Medigap plan typically covers the rest, so most members pay $0.

Medicare Advantage

Four Louisiana Rules and Programs Worth Knowing

Medicare is federal, but the rules and assistance programs around it change at the state line. These four are specific to Louisiana, and your advocate works with them every week.

Medigap: a yearly birthday window to switch

Louisiana adds a state right on top of federal Medigap law. Medigap is regulated by the Louisiana Department of Insurance, and under Louisiana Revised Statutes 22:1112 anyone who already has a Medigap policy gets a 63-day window every year starting on their birthday to switch to a plan with equal or lesser benefits from the same insurer or one of its affiliates, with no health questions and no health-based price. Outside that window and the federal guaranteed-issue rights, a switch is medically underwritten. Under 65, people on Medicare through disability get the same six-month Medigap open enrollment as people turning 65, starting with Part B (Department of Insurance Regulation 33): every plan the insurer sells must be offered regardless of age, though premiums run higher. Questions and complaints go to the Department of Insurance at 1-800-259-5300.

LaSHIP: free state Medicare counseling

Louisiana's State Health Insurance Assistance Program is LaSHIP, the Louisiana Senior Health Information Program, run by the Louisiana Department of Insurance (it was called SHIIP until 2025). Counselors are free and unbiased, and they help with plan comparisons, Medicare Savings Program applications, and Extra Help. Statewide line: 1-800-259-5300. Great for one-off enrollment questions; we point our own members there during Open Enrollment. For local services such as meals, rides, and in-home help, every parish has a Council on Aging, coordinated by the Governor's Office of Elderly Affairs ((225) 342-7100, or the national Eldercare Locator, 1-800-677-1116). For nursing home and assisted living concerns, the Louisiana Long-Term Care Ombudsman Program answers at 1-866-632-0922.

Medicare Savings Programs with no asset test

Louisiana does not count savings or property for its Medicare Savings Programs: the Louisiana Department of Health (Louisiana Medicaid) disregards resources entirely for QMB, SLMB, and QI, so eligibility turns on income alone. QMB (income up to 100% of the federal poverty level) pays the Part B premium plus Medicare deductibles and coinsurance, and providers are barred by law from billing QMB members for Medicare cost-sharing; SLMB and QI pay the Part B premium. Any level automatically qualifies you for Part D Extra Help. Louisiana has no state pharmaceutical assistance program; SenioRx, offered through the parish Councils on Aging, helps people apply to drug-company assistance programs rather than paying for drugs itself. Apply online through the Louisiana Medicaid self-service portal, by phone at 1-888-342-6207, or with help from LaSHIP. Most eligible seniors never claim these, and screening for every one of them is what an advocate does first.

Home care, paid family caregiving, and PACE

Home care for older adults runs through the Louisiana Department of Health's Office of Aging and Adult Services, and every program starts with a call to Louisiana Options in Long Term Care at 1-877-456-1146. The Community Choices Waiver serves people 65 and older, or 21 to 64 with a physical disability, who need a nursing-home level of care (there is a waiting list); a relative such as an adult child can be the paid personal-assistance worker, including through the self-direction option, while a spouse can be paid only with the office's extraordinary-care approval and the responsible representative cannot be paid. Long Term-Personal Care Services is stricter and does not pay a spouse, legal guardian, or responsible representative. Louisiana expanded Medicaid in 2016, and it has four PACE programs, in the New Orleans, Baton Rouge, Lafayette, and Alexandria areas, for people 55 and older. For dual-eligible families we handle the Medicare side while you run the caregiving.

Yes, during your first six months with Part B. Under Louisiana Department of Insurance Regulation 33, people who get Medicare through disability have the same six-month Medigap open enrollment period as people turning 65, starting the month Part B begins: every Medigap plan an insurer sells must be offered to you regardless of age, and it cannot be denied or priced higher because of your health. Premiums for under-65 buyers are usually much higher, though, and an insurer may exclude a pre-existing condition for up to six months unless you had prior creditable coverage. Miss that window and coverage is generally medically underwritten until a new federal right applies. Questions go to the Louisiana Department of Insurance at 1-800-259-5300, the same line as LaSHIP.
