Statewide, 37.4% of beneficiaries chose Medicare Advantage in 2025. Counties above that line see more prior authorizations and more denials worth appealing; counties below it lean on Medigap, where South Dakota has no birthday rule but does give people on Medicare because of a disability the same six-month Medigap open enrollment as people turning 65. About 9.3% of South Dakota seniors live below the poverty line, the group most likely to qualify for unclaimed help: federal Extra Help and South Dakota's Medicare Savings Programs, which in 2026 reach monthly incomes under $1,816 for one person or $2,455 for a couple.
